Job Summary
As a Mid-Level Provider at Sheldon Health Hub, you’ll help people stay healthy and care for those who are sick. You’ll work with doctors, nurses, and other healthcare team members to make sure patients feel well cared for and understood. You’ll talk with patients about their health, listen to their concerns, and help them feel better. You’ll also help patients understand ways to stay healthy and manage their health.
What You’ll Do (Your Responsibilities)
Every day, you’ll have important tasks that help patients and their families. Here’s what you can expect to do in this role:
Checking on Patients
- Look after patients by listening to their health concerns and learning about their medical history.
- Do physical exams to check their health.
- Order and review tests (like blood tests or X-rays) to understand what the patient needs.
Giving Care and Treatment
- Diagnose health problems to find out what is making patients feel unwell.
- Create treatment plans with medicines or health advice that help patients get better.
- Give clear instructions to help patients understand their treatment and why it’s important.
Teaching Patients About Health
- Educate patients and families on how to care for themselves and make healthy choices.
- Explain lifestyle tips (like exercise and healthy eating) to help patients stay well and manage their health better.
Working with the Healthcare Team
- Work closely with other healthcare team members like doctors and nurses to give the best care.
- Join team meetings to talk about clinic goals and improve patient care.
Keeping Records
- Record patient details carefully to keep accurate health information.
- Follow patient privacy rules to protect personal information.
What You Need to Apply
Education and Experience
- Nurse Practitioner (NP) or Physician Assistant (PA) License in Texas.
- At least 2 years of healthcare experience (such as in a community clinic or family health center).
- Certification from a respected NP or PA organization (such as AANP or NCCPA).
Important Skills
- Good communication skills to talk with patients and their families.
- Understanding of healthcare basics and how to promote good health.
- Experience with electronic records to keep track of patient information.
Extra Skills That Can Help
- Being bilingual, especially if you speak Spanish, to help our diverse community.
- Experience in public or community health to understand the needs of local patients.
- Knowledge of trauma-informed care, which means understanding the emotional needs of patients who have experienced difficult situations.
